Exchange Back Pressure

Are any of you monitoring for Exchange Back Pressure?  I am thinking about trying to build a service for this because its a combination of multiple things that can come into play and cause such an issue.  It's complicated though so I figured I would ask here.  I also haven't used the service module yet to know of any limitations.
