you've read https://www.logicmonitor.com/support/settings/netscan-policies/creating-netscan-policies-copy
your csv has both IP and displayName headers

Are there any other fields in your csv?
Are you passing in multiple properties?
Is there a carriage return after the last line (shouldn't be needed, but might be)?
