Include ##INSTANCE## on graphs for vCenter

When creating graphs for ESX servers or the vCenter server automatically include the ##INSTANCE## token n the names of all the graphs. Currently, they either get nothing but the metric (CPU, Mem, etc). In the Dashboard, the title is <>-ESX Virtual Machine --<> which makes it hard to correlate the graph to the Virtual Machine for the metric.

We are also missing the INSTANCE in the name of the exported data file:rnanpdfil2-NetAppVol-Usage-2012-05-20_02-20_to_2013-05-19_23-00_PDT.csv
